This Patient was not pleased with how her jaw line was increasingly becoming "square shaped" as she was aging. An excellent example of how subtle changes can make big differences. This patient desired more definition along her jaw line, cheekbone and neck. Her needs were very subtle and her desire was to keep from having "a more drastic procedure" in the future. This case presented a unique challenge in that given the type of skin quality, moderate to severe jowling and neck fullness the initial surgical recommendation was for a composite procedure under general anesthesia. The patient however was firmly against "going under". Therefore a facelift procedure combined with platysmaplasty was performed under local anesthesia obtaining excellent results in all areas of concern. Key to successful outcome was meticulous work in the submental area (under the chin). This case illustrates that in experienced hands dramatic and long lasting results can be achieved using only local anesthesia. Additional Recommendations: Chemical peel, Fillers
